Keratitis of eye cornea

Keratitis, or inflammation of the cornea, is one of the diseases that occur frequently in the practice of ophthalmologists.

Normally cornea is transparent, structural and optically homogeneous. It has a perfect spherical surface. The inflammatory process alters the structure of cornea and may cause its persistent opacity, which is accompanied by decrease of vision.

Causes

Keratitis can be caused by exposure of both external and internal factors. Very often the cause of corneal inflammation are infections (pathogenic bacteria, fungus, viruses, protozoa). Keratitis often occurs due to exposure of mechanical (injury, wearing contact lenses), chemical, thermic factors. Among the internal causes of keratitis should be noted allergic diseases (pollinosis, hay fever), systemic pathologies (diabetes mellitus, connective tissue diseases).

Symptoms

Inflammation of the cornea is accompanied by the appearance of so-called "corneal syndrome", which includes: lacrimation, blepharospasm and photophobia. The patient with keratitis is experiencing severe discomfort, mucous membrane of the affected eye is swollen, hyperemic, trying to open eyes causes intense pain.

In addition to corneal syndrome different types of keratitis have its own expression. The severity of pathological changes depends on the state of general and local immunity, peculiarities of the pathogen, etc. With the unfavorable course of the disease the inflammation may spread to deeper structures of the eye. Ulceration of the cornea is also possible, the accession of secondary infection.

Diagnostics

Symptoms of keratitis are quite typical, so we can diagnose the disease at the stage of collection of anamnesis and examination of the patient. To clarify the diagnosis (localization, extent and depth of lesion) is used the slit lamp examination (biomicroscopy of the eye). The probe with fluorescein allows to identify ulceration of the cornea . During the survey for clarification of further treatment tactics, the doctor may take a smear from the affected area, followed by sowing on nutrient media, to determine the susceptibility of inflammation to antibiotics.

At the examination of the patient with keratitis on the corneal surface are determined single or multiple inflammatory infiltrates (accumulation of leukocytes and lymphocytes), blood vessel ingrowth (vascularization of cornea). At a superficial lesion infiltrates may dissolve almost completely, without disturbing the transparency of the cornea. The outcome of deep keratitis can be rough scarrings of the corneal tissue, which lead to permanent clouding of the cornea and cause decrease of vision.

At herpetic keratitis in the surface layers of the cornea are revealed tiny bubbles, which, after spontaneous opening leave erosions or ulceration of characterised shape.

Complications

One of the unfortunate options of cornea lesion of infectious nature is creeping ulcer of the cornea. Its reason usually is pathogenic coccus, and chronic conjunctivitis or microtrauma of the cornea contributes to its appearance. At examination is revealed local purulent fusion of tissues, which in case of progression, leads to perforation of the cornea with the outcome as a rough scar (leucoma). Due to spreading of inflammation in creeping ulcer of the cornea endophthalmitis may be developed.т.

In case of erosion or ulceration of the cornea are prescribed medications stimulating epithelization of the cornea, physiotherapy (electrophoresis and phonophoresis of drugs). Patients with the threat of perforation or rough scarring changes of the cornea and persistent reduction of visual acuity may need corneal transplantation (keratoplasty).
