Scleral sealing

Episcleral buckling surgery today is distributed very widely due to the simplicity, safety and high efficiency of this operation. During the surgery is usually used fine-meshed silicone sponge as sealing material. The seal is seamed to eyeball from the outside and the sides of the eye are pressed to bring the choroid close to the detached retina, this helps to block the detachment in the cavity shaft. After that a break in the retina can be restricted by cryopexy or laser coagulation.

Depending on the area of cavity and location of seals, episcleral buckling surgery is a local (radial, sectoral) or circular. When retinal detachment, episcleral surgery is performed as a standalone surgery method or in conjunction with endovitreal intervention.

Radial sealing of the sclera is performed in case of isolated ruptures of the retina with its fresh local perifocal detachment.

Sectorial sealing is prescribed in case of several nearby breaks or giant retinal breaks with detachment from the serrated line, when the shaft cavity shall have a greater intensity and length.

Circular sealing is recommended in severe cases, like isolated rupture of the retina, but with extensive areas of its peripheral degeneration or multiple retinal tears with involvement of multiple quadrants. In case of retinal detachment with severe tractions on the retina of the vitreous body, coupled with extensive areas of the peripheral dystrophy and previously unrecognized retinal breaks, is applied circular sealing with silicone band or circular sealing with porous silicone wisp.

Process of the operation

Before surgery the location of retinal detachment is determined, then is produced the required size seal. Today the main material for seals is a soft silicone sponge, which is flexible and easy to use. During the operation (under drip anesthesia or narcosis) the conjunctiva is cut and the seal is imposed on the sclera, then it is fixed with the seams. If necessary, drainage of the accumulated fluid is performed, with its removal through the special opening. Some cases of surgical intervention require the introduction into the cavity of the eyeball of special expanding gas or air. The last stage of the surgery is imposition of compressing sutures on the conjunctiva.

Efficiency

After the surgery, the restoration of vision occurs gradually, and usually lasts for 2-3 months. Sometimes this process can extend to half a year, due to older age of patients or presence of myopia.

Extra scleral filling, as a rule, does not lead to full recovery of vision. Its maximum level depends on the term of the detachment and degree of involvement in the process of the retinal central part responsible for visual acuity.
