PTK (Phototherapeutic keratectomy)Excimer laser surgeries, prescribed for the pathologies of the cornea, are divided into medical, cosmetic, optical-reconstructive, mixed and combined.

The goal of excimer laser treatment interventions is acceleration of recovery of the epithelial or stromal ulcerations, removal of corneal syndrome. This is achieved by removal of the affected epithelium, and corneal tissue stroma of the inflamed eye.

Optical-reconstructive surgeries consider simultaneously performed optical and reconstructive manipulations, for example, with excision of mixed subepithelial opacities that occur on the surface of the corneal stroma after earlier performed refractive interventions.

Specialties of the operation

Excimer laser cosmetic keratectomy is advisable to carry out to improve the external condition of the anterior eye area when having corneal leukoma in eyes with low residual vision, or blind eyes.

Excimer laser mixed operations in corneal pathology include various combinations of medical, cosmetic and optical-reconstructive procedures, the execution of which is carried out with the aim of eliminating corneal syndrome with simultaneous achievement of optical, refractive, optical-refractive or cosmetic effects.

The main difference between the excimer laser combined interventions, is that during the operation both laser and microsurgical manipulations are applied which is necessary to achieve the best therapeutic or optical-reconstructive result.

When describing in foreign sources excimer laser operations at pathologies of the cornea, most often are used following terms: photorefractive keratectomy (PRK) and phototherapeutic keratectomy (PTK).

First PTK surgery was performed in 1985, and since then, this method has been widely used in the treatment of various corneal pathologies: dystrophies and degenerations, post-traumatic scars, recidivous erosions, irregularity, opacities after keratitis.

Excimer laser PTK is especially effective in removing of superficial corneal opacities, the size of which is less than one third of its thickness. However, there is a competent opinion of the ophthalmologists that this operation is also justified at clouding, uneven in depth and catching different areas up to one-half of the thickness of the corneal stroma.

Excimer laser keratectomy (partial or full) is effective at intolerable to conservative treatment ulcerative lesions of the cornea of bacterial or viral etiology. Regardless the etiology of pathological process that caused corneal opacity, optical-reconstructive excimer laser keratectomy is performed at the corneal opacities that capture 1/4-1/2 of the corneal stroma, while the original thickness of it in the central optical zone is more than 430 µm.

In medical rehabilitation of patients, equally important are the problems of refractive errors correction, often combined with corneal opacities. Thus, special importance is attached to surgical procedures that eliminate irregular astigmatism. The application of the rotational and scanning systems used in excimer laser installations makes even distribution of the energy density over the entire area of the ablation zone possible. It provides a uniform transition of the ablation zone, with simultaneous refractive profiling of the corneal surface.

Today there is a wide variety of excimer laser intervention modifications aimed at correction of induced ametropia. But they all are based on three basic operations – PRK, PTK and LASIK.

Specialists of our clinic have successfully mastered all three techniques of excimer laser interventions, including PTK (phototherapeutic keratectomy), with help of which is executed an effective treatment of corneal opacities and its dystrophies.
