VitrectomyVitrectomy (vitreum – vitreous body, ectome - cut, delete) - ophthalmologic surgery of partial or complete removal of the changed vitreous body.

This intervention helps to restore the transparency of the vitreous body and eliminate tension in the tissues of the retina area in order to prevent its detachment.

The operation requires special microsurgical equipment, the intervention is performed by vitreoretinal surgeon.

Indications for vitrectomy

There is a number of pathologies that have indications for removal of the vitreous body:

  • massive hemorrhage in the vitreous body cavity (total and subtotal hemophthalmus, recurrent partial hemophthalmus) with no effect of drug therapy
  • severe proliferative vitreoretinopathy with the formation of dense fibrous strands in the vitreous body, fixed retina folds, neovascularization
  • large tears or retinal detachment
  • expressed vitreous body opacities
  • foreign bodies of the vitreous body
  • penetrating eye injuries or surgeries, accompanied by a significant loss of the vitreous body

In addition, vitrectomy can be performed to ensure sufficient access for surgical interventions on the posterior chamber of the eyeball.

Contraindications

Vitrectomy - a complex abdominal surgery, which has a number of contraindications. The intervention is not performed on patients with severe corneal clouding, severe disorders of blood coagulation, the tendency to allergic reactions.

Technique

At the discretion of the attending specialistsician, vitrectomy may be performed under general or local anesthesia. After dilution and fixation of the eyelids using blepharostat on the sclera of the eyeball three small incisions are made. Through these incisions, the doctor inserts into the cavity of the eyeball special microsurgical instrument vitreotom, as well as illuminant and infusion cannula. Vitreous body gets separated and drained, pathologically diseased tissues that cause the tension of the retina get removed, scars and fibrotic bands get dissected.

After that, the cavity is introduced with substitute (silicone oil, gas mixture, PFOS). Due to filling the vitreous cavity with substitute, a tight contact with the underlying tissues of the retina is created, which helps to keep the retina in the physiological state, avoiding repeated intraocular bleeding, prevent the growth of abnormal blood vessels (neovascularization).

Duration of vitrectomy surgery can vary significantly depending on the state severity of the patient, and is approximately 1.5 to 3.2 hours. In particularly difficult cases, it may take a few interventions.

After the operation the patient for some time will be recommended to avoid heavy lifting, intense physical and visual loads, fast slopes etc.

Possible complications

As any abdominal surgery, vitrectomy has some risk of complications that may arise in the course of the operation (bleeding, infection, retinal detachment, damage to the lens) and thereafter (corneal edema, increased intraocular pressure, repeated hemorrhage).
